1099-HC Forms Arriving this Month!

Please check your U.S. Mail at home for your 1099-HC forms.  If you participated in either/or both of our Medical Plans in 2018 (Blue Cross and/or Tufts), the providers will be sending 1099-HC forms to your home.  You'll need these forms when filing your 2018 MA Income Tax.

Employee Self-Service Payroll Information

Some exciting news from the Payroll Office!  The town set up an Employee Self Service website that provides payroll information for all employees.   

The Employee Self Service page is a snapshot of what is currently in MUNIS.  At this time we only programmed the Employee Master screen, which will show you what your current rate of pay is, what your accrual balances are for Sick, Vacation and Personal time, your demographic information such as address and phone numbers, and what you are claiming for exemptions for Federal and State taxes.  You will also be able to view and print your check stubs back to the inception of MUNIS and see your W2!

Your User name will always be your first initial, last name, and last four of your SS#

          ie)  jsmith1234

Your temporary password is the last four of your social security number. You will be prompted to change that password when you first log in.

When you log in and change your password, click 'Employee Self Service' on the left to see your account information.

●     Submit employment applications and/or resumes or  verify that your resume was received through our electronic submission process;
●     Submit a Recommendation to Hire for processing, and/or check on the status of an offer letter for a new employee, including coaching and stipend positions, Extended School Year (ESY) and temporary summer help positions;
●     Submit all new hire paperwork for processing;
●     Submit a CORI application, and obtain information about fingerprint-based background checks and how/where to get them done;
●     Check on the status of annual assignment letters for existing employees, or report an error in your assignment letter;
●     Submit Course Approval RequestsTuition Reimbursement/Course Credit Requests, Intent to Submit forms for processing and/or check on the status of your Course Approval Request,  and/or Tuition Reimbursement/Course Credit Request;
●     Request an appointment to view your personnel file;
●     Submit Family & Medical Leave Act (FMLA) forms for processing.

●     Make additions or changes to benefit plans, such as:  Medical, Dental, Life, Long Term Disability and Flexible Spending Accounts during open enrollment or          if you experience a qualifying event that allows you to enroll/change outside of the open enrollment period;
●     Find out about our Wellness Program benefits, such as Weight Watchers at Work and other events;
●     Submit enrollment forms for MA Teachers, Middlesex County, or OBRA mandatory retirement plans;
●     Submit enrollment and salary reduction agreements for voluntary 403b and 457 retirement plans;
●     Submit Accident Report forms for potential Workers’ Compensation benefits;
●     Get information about annual Open Enrollment for benefit plans.
