What medical documentation do you need for coming to school?
Medical documentation and vaccine records are required for enrollment and prior to starting school in Hopkinton Public Schools per the Hopkinton School Committee Policy, in conjunction with Massachusetts state law. There are exceptions for this documentation for homeless children in foster care and other groups outlined in the links below.

School Screenings
The state requires different screenings throughout a student’s education to make sure they are able to learn and participate safely. These screenings also help to catch changes in a student, which if unfound, will make learning more difficult. These screenings are done by nurses in the school and sometimes in collaboration with the physical education classes.
If you can opt out your student from screenings, a google form has been sent out to families already this year (2022-23 school year)
You will not hear from the school unless your student has a screening outside of the expected range, or if they are . If there is a need for follow up you will be notified to follow up with your doctor or specialist.
In PreKindergarten the screenings are:
-
Vision
In Kindergarten the screenings are:
-
