MEDICATION & ALLERGIES
Does your child have an allergy?
It is very important that we know about all allergies to keep our students safe. If you have an allergy action plan which includes medication, we will need it.
Does your child need a medication administered at school?
You will need to provide the following so the clinic can give a medication:
Please keep your student’s PowerSchool account up to date with allergies.

JLCD-FRM1 - Hopkinton Public School District The top part of the JLCD-R1 form has to be completed, signed, and dated by parents. A doctor’s office will fill out the bottom portion.

Medications must be in their original pharmacy labeled containers. No medication may be dispensed without written authorization from a physician, on the appropriate medication administration form, which may be obtained from the school nurse or online. An adult must deliver medications to school. No medications can be brought to school by a student. All medications must be picked up by a parent or guardian at the end of the school year. Those not picked up, will be disposed of properly on the last day of school.
School Screenings
Annual In School Screenings at Hopkins School
The state requires different screenings throughout a student’s education to make sure they are able to learn and participate safely. These screenings also help to catch changes in a student, which if unfound, could make learning more difficult. These screenings are done by nurses in the school clinic and sometimes in collaboration with the physical education classes.
You will not hear from the school unless your student has a screening outside of the expected range. If a screening is out of range, we will recommend follow-up with a doctor for referral to the appropriate resource.
- Students in grade 4 will have screenings by the nurse for vision, and height and weight to calculate BMI (Body Mass Index)
- Students in 5th grade will have annual screening by the nurse for Vision and Postural: vision screening and a postural screening to check for scoliosis.
Registration
Requirements for being a registered student at Hopkins school:

Annual physical exam with ability to participate in school activities documented within 12 months of the start of the school year.. This is required upon registration into the school system, no matter the age, and also in grades: K, 4, 7, and 10.
Immunization Requirements for School Entry
Grades 4 and 5
DTap: 5 doses; 4 doses are acceptable if the fourth dose is given on or after the 4th birthday. DT is only acceptable with a letter stating a medical contraindication to DTaP
Polio: 4 doses; fourth dose must be given on or after the 4th birthday and ≥6 months after the previous dose, or a fifth dose is required. 3 doses are acceptable if the third dose is given on or after the 4th birthday and ≥6 months after the previous dose
Hepatitis B: 3 doses; laboratory evidence of immunity acceptable
MMR: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; laboratory evidence of immunity acceptable
Varicella: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; a reliable history of chickenpox* or laboratory evidence of immunity acceptable
Influenza vaccine yearly: This is highly recommended, not currently required.
