MEDICATION & ALLERGIES
Does your child have an allergy?
It is very important that we know about all allergies to keep our students safe. If you have an allergy action plan which includes medication, we will need it.
Does your child need a medication administered at school?
You will need to provide the following so the clinic can give a medication:
Please keep your student’s PowerSchool account up to date with allergies.

Medications must be in their original pharmacy labeled containers. No medication may be dispensed without written authorization from a physician, on the appropriate medication administration form, which may be obtained from the school nurse or online. An adult must deliver medications to school. No medications can be brought to school by a student. All medications must be picked up by a parent or guardian at the end of the school year. Those not picked up, will be disposed of properly on the last day of school.

Middle School Requirements for School Entry

Annual Physical Exam Required within 12 months before starting any grade or within 30 days of beginning school in the district.

Annual Physical Required within 12 months of starting 7th grade.

Annual Physical Required within 12 months of beginning any grade to participate in school sponsored sports.
VACCINES:
Grade 6:
DTap: 5 doses; 4 doses are acceptable if the fourth dose is given on or after the 4th birthday. DT is only acceptable with a letter stating a medical contraindication to DTaP
Polio: 4 doses; fourth dose must be given on or after the 4th birthday and ≥6 months after the previous dose, or a fifth dose is required. 3 doses are acceptable if the third dose is given on or after the 4th birthday and ≥6 months after the previous dose
Hepatitis B: 3 doses; laboratory evidence of immunity acceptable
MMR: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; laboratory evidence of immunity acceptable
Varicella: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; a reliable history of chickenpox* or laboratory evidence of immunity acceptable
†Medical exemptions (statement from a physician stating that a vaccine is medically contraindicated for a student) must be renewed annually at the start of the school year and religious exemptions (statement from a student, or parent/guardian if the student is <18 years of age, stating that a vaccine is against sincerely held religious beliefs) should be renewed annually at the start of the school year.
Grades 7 – 12†
TDap: 1 dose; and history of DTaP primary series or age appropriate catch-up vaccination. Tdap given at ≥7 years may be counted, but a dose at age 11-12 is recommended if Tdap was given earlier as part of a catch-up schedule. Td or Tdap should be given if it has been ≥10 years since last Tdap

Meningococcal Requirements
Grade 7-9: 1 dose; 1 dose MenACWY (formerly MCV4) required. Meningococcal B vaccine is not required and does not meet this requirement.
Grade 11-12: 2 doses; second dose MenACWY (formerly MCV4) must be given on or after the 16th birthday and ≥ 8 weeks after the previous dose. 1 dose is acceptable if it was given on or after the 16th birthday. Meningococcal B vaccine is not required and does not meet this requirement.

Massachusetts school immunization requirements are created under authority of 105 CMR 220.000 Immunization of Students Before Admission to School
Requirements apply to all students including individuals from another country attending or visiting classes or educational programs as part of an academic visitation or exchange program. Requirements apply to all students, even if over 18 years of age.

Medical Physicals within 12 months of starting school are required for initial registration to the District, no matter what year they start, and also in grades K,4,7,& 10. The physical exam form should include the clearance or restrictions for participation in school activities listed.

If your student participates in school sponsored sports, you will need to provide a physical exam form every year.

School Screenings
The state requires different screenings throughout a student’s education to make sure they are able to learn and participate safely. These screenings also help to catch changes in a student, which if unfound, will make learning more difficult. These screenings are done by nurses in the school clinic and sometimes in collaboration with the physical education classes.
You will not hear from the school unless your student has a screening outside of the expected range. You will then be referred to follow up with your pediatrician.

As required by the Dept of Public Health, all students must be screened at least once using the S-BIRT tool. This acronym stands for Screening, Brief Intervention, and Referral to Treatment (SBIRT.)
SBIRT is a comprehensive, integrated, public health approach to the delivery of early intervention and treatment services for persons with substance use disorders, as well as those who are at risk of developing these disorders.
