Does your child have an allergy?  

 

It is very important that we know about all allergies to keep our students safe.  If you have an allergy action plan which includes medication, we will need it.

 

Does your child need medication administered at school? You will need to provide the following so the clinic can give a medication:

• A copy of the Physician Order for the medication

• Allergy Action Plan from your physician, if applicable

• Medication Policy

Medications must be in their original pharmacy labeled containers. No medication may be dispensed without written authorization from a physician, on the appropriate medication administration form, which may be obtained from the school nurse or online. An adult must deliver medications to school. No medications can be brought to school by a student. All medications must be picked up by a parent or guardian at the end of the school year. Those not picked up, will be disposed of properly on the last day of school.

 

Admission Requirements for registration and continued enrollment in Hopkinton Public Schools

Medical documentation and vaccine requirements for Enrollment in Hopkinton Public Schools are per the Hopkin School Committee Policy.

 

Requirements apply to all students including individuals from another country attending or visiting classes or educational programs as part of an academic visitation or exchange program. Requirements apply to all students, even if over 18 years of age.

 

• Medical Physicals within 12 months of starting school are required for initial registration to the District, no matter what year they start, and also in grades  K,4,7,& 10.  The physical exam form should include the clearance or restrictions for participation in school activities listed.

• If your student participates in school sponsored sports, you will need to provide a physical exam form every year.

Vaccine Requirements: 

Grades 7 – 12

TDap: 1 dose; and history of DTaP primary series or age appropriate catch-up vaccination. Tdap given at ≥7 years may be counted, but a dose at age 11-12 is recommended if Tdap was given earlier as part of a catch-up schedule. Td or Tdap should be given if it has been ≥10 years since last Tdap

Polio: 4 doses; fourth dose must be given on or after the 4th birthday and ≥6 months after the previous dose, or a fifth dose is required. 3 doses are acceptable if the third dose is given on or after the 4th birthday and ≥6 months after the previous dose

Hepatitis B: 3 doses; laboratory evidence of immunity acceptable.  2 doses of Heplisav-B given on or after 18 years of age are acceptable

MMR: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; laboratory evidence of immunity acceptable

Varicella: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; a reliable history of chickenpox* or laboratory evidence of immunity acceptable

Influenza Annual Vaccine: Not required, but recommended. 

 

 Meningococcal Requirements

Grade 7-9: 1 dose; 1 dose MenACWY (formerly MCV4) required. Meningococcal B vaccine is not required and does not meet this requirement.

Grade 11-12: 2 doses; second dose MenACWY (formerly MCV4) must be given on or after the 16th birthday and ≥ 8 weeks after the previous dose. 1 dose is acceptable if it was given on or after the 16th birthday. Meningococcal B vaccine is not required and does not meet this requirement.

• As required by the Dept of Public Health, all students must be screened at least once using the S-BIRT tool during their time in High School.  This acronym stands for Screening, Brief Intervention, and Referral to Treatment (SBIRT.) SBIRT is a comprehensive, integrated, public health approach to the delivery of early intervention and treatment services for persons with substance use disorders, as well as those who are at risk of developing these disorders.
