Lisa has been a nurse for 26 years, with experience in Pediatrics, Cardiac, Labor & Delivery, and Community Health. She has a Master's in Nursing with a speciality in Public Health, and is nationally certified, as a School Nurse. Lisa has three children, and an adorable Mini Labradoodle named Teddy. She is passionate about promoting independence and self awareness with each student's personal health.

Does your child have an allergy?
It is very important that we know about all allergies to keep our students safe. If you have an allergy action plan which includes medication, we will need it.
Does your child need medication administered at school? You will need to provide the following so the clinic can give a medication:

Medications must be in their original pharmacy labeled containers. No medication may be dispensed without written authorization from a physician, on the appropriate medication administration form, which may be obtained from the school nurse or online. An adult must deliver medications to school. No medications can be brought to school by a student. All medications must be picked up by a parent or guardian at the end of the school year. Those not picked up, will be disposed of properly on the last day of school.

Massachusetts school immunization requirements are created under authority of 105 CMR 220.000 Immunization of Students Before Admission to School
Requirements apply to all students including individuals from another country attending or visiting classes or educational programs as part of an academic visitation or exchange program. Requirements apply to all students, even if over 18 years of age.

Medical Physicals within 12 months of starting school are required for initial registration to the District, matter what year they start, and grades K,4,7,& 10. The physical form clearance or restrictions for participation in school activities listed.
Vaccine Requirements for Elmwood:
DTap: 5 doses; 4 doses are acceptable if the fourth dose is given on or after the 4th birthday. DT is only acceptable with a letter stating a medical contraindication to DTaP
Polio: 4 doses; fourth dose must be given on or after the 4th birthday and ≥6 months after the previous dose, or a fifth dose is required. 3 doses are acceptable if the third dose is given on or after the 4th birthday and ≥6 months after the previous dose
Hepatitis B: 3 doses; laboratory evidence of immunity acceptable
MMR: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; laboratory evidence of immunity acceptable
Varicella: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; a reliable history of chickenpox* or laboratory evidence of immunity acceptable

Attendance Policy
School Screening
The state requires different in-school screenings throughout a student’s education to make sure they are able to learn and participate safely. These screenings also help to catch changes in a student, which if unfound, will make learning more difficult. These screenings are done by nurses in the school clinic and sometimes in collaboration with the physical education classes.
You will not hear from the school unless your student has a screening outside of the expected range. You will then be referred to follow up with your pediatrician.
