·    Evaluator responsible for the blue shaded areas

·    Educator responsible for the green shaded areas

·    District / Principal responsible for the orange shaded areas

·    Evaluator & Educator* share responsibility for purple shaded areas

 
* Evaluators are responsible for meetings; however educators should prepare evidence and information well in advance of completion dates.

Evaluator meets with first-year educators to assist in self-assessment and goal setting process (“first year” refers to any educator in their first year in the district; therefore, all Pre-PTS educators in year one.)

Educator submits self-assessment

Educator submits proposed goals and educator form (Educator Plan may be established at Summative Evaluation Report meeting in prior school year)

Evaluator completes Educator Plans

Evaluator should complete first observation of each Educator

November 15 (1 year plans)

Educator on one year plan submits evidence on parent outreach, professional growth, progress on goals (and other standards, if desired) for mid-cycle Formative Assessment

* or 4 weeks before Formative Assessment Report date established by Evaluator

Evaluator should complete mid-cycle Formative Assessment Reports for Educators on one-year Educator Plans

Evaluator holds Formative Assessment Meetings if requested by either Evaluator or Educator

Educator submits evidence on parent outreach, professional growth, progress on goals (and other standards, if desired) for Formative or Summative Evaluation

April 20*

*or 4 weeks prior to Formative or Summative Evaluation Report; date established by evaluator

Observations required by Educator Plan completed (additional observations may be conducted after this date)

Evaluator completes Summative Evaluation Report

May 15 (1 and 2 Year Plans)

Evaluator meets with Educators whose overall Summative Evaluation ratings are Needs Improvement or Unsatisfactory

Evaluator meets with Educators whose ratings are proficient or exemplary at request of Evaluator or Educator

June 10

Educator signs Summative Evaluation Report and adds response, if any, within 5 school days of receipt
